 Onderstaande doseringen gelden voor de behandeling van ernstige infecties met stafylokokken (al dan niet in combinatietherapie met een beta-lactam). 		Comments:
 Daptomycine wordt beschouwd als reservemiddel/middel met een specialistisch indicatiegebied. Voorschrijven dient uitsluitend plaats te vinden op advies van een arts-microbioloog of internist-infectioloog.

Notes GFR general:

De toepassing van high-dose daptomycine (>8 mg/kg) is niet uitvoerig onderzocht bij patiënten met verminderde nierfunctie/nierfunctie-vervangende therapie. Monitor nauwlettend op bijwerkingen.

Renal function-replacement therapy

CAPD: Zie GFR <30 mL/min.

Hemo dialysis: Zie GFR <30 mL/min (op dialysedagen toedienen na dialyse). Er is onvoldoende ervaring met het geven van 150% van de keerdosering (15-18mg/kg) bij een dialyse-interval van 72 uur. Veiligheidshalve kan gekozen worden voor een dialyseschema van elke 48u.

General comments

Bovengenoemde doseringen zijn hoger dan de geregistreerde dosering. Het risico op (ernstige) bijwerkingen (waaronder eosinofiele pneumonie, myopathie) neemt toe met de daptomycine blootstelling. Monitor nauwlettend op deze bijwerkingen.

Spiegelbepalingen (TDM)
Spiegelbepaling van daptomycine is niet mogelijk. 

(Morbide) obesitas
Er zijn weinig gegevens over de toepassing van high-dose daptomycine (>8 mg/kg) bij (morbide) obese patiënten. Bij patiënten met een BMI van 30 kg/m2 of hoger kan veiligheidshalve gekozen worden om te doseren op Adjusted Body Weight (gewichttype 'Aangepast' in het EPD). 

ECLS/ECMO
Er zijn weinig gegevens over de toepassing van daptomycine bij ECMO. Er wordt op basis van beperkte gegevens geen dosisaanpassing geadviseerd.

Verminderde leverfunctie
Er wordt geen dosisaanpassing geadviseerd in geval van verminderde leverfunctie.
