Dosage:
 Eravacycline wordt beschouwd als reservemiddel/middel met een specialistisch indicatiegebied. Voorschrijven dient uitsluitend plaats te vinden op advies van een arts-microbioloog of internist-infectioloog. 		Comments:
 De dosering hoeft niet te worden aangepast in geval van verminderde nierfunctie/nierfunctie-vervangende therapie.

Renal function-replacement therapy

CAPD: Geen dosisaanpassing nodig.

Hemo dialysis: Geen dosisaanpassing nodig. Eravacycline kan worden toegediend zonder rekening te houden met het tijdstip van hemodialyse.

CAV / VVHD: Geen dosisaanpassing nodig.

General comments

Aandachtpunten/aanvullende monitoring
Het gebruik van eravacycline is geassocieerd met een verhoogd risico op hypofibrogenemie en andere coagulopathieën. Bij gebruik >7 dagen dagen wordt geadviseerd om (naast reguliere labcontrole) wekelijks INR/PT en fibrinogeen te bepalen.  

Spiegelbepalingen (TDM)
Spiegelbepaling van eravacycline is niet mogelijk.

(Morbide) obesitas
Eravacycline wordt gedoseerd op (werkelijk) lichaamsgewicht. 

ECLS/ECMO
Op basis van beperkte data wordt geen klinisch relevant effect op de plasmaconcentraties van eravacycline verwacht en wordt geen dosisaanpassing geadviseerd.

Verminderde leverfunctie
Er is geen dosisaanpassing nodig voor patiënten met een leverfunctiestoornis.
