	Medication: cefazoline iv loading dose 2000mg, followed by cefazoline iv 6000mg continuous per 24 hours	Remarks: Ook bij penicilline-allergie mild (NIET IgE gemedieerde (type I) en NIET Stevens-Johnson syndroom/TEN) BMI>40: cefazoline iv oplaaddosis 3000mg, gevolgd door cefazoline iv 9000mg continue per 24 uur
