Spiegelbepalingen (TDM)

Op indicatie is het mogelijk om spiegelbepalingen van caspofungine uit te voeren in overleg met de ziekenhuisapotheker, medisch microbioloog en/of infectioloog.

(Morbide) obesitas

CAM vergadering 25-1-2021: Na discussie met Märtson (zie bronnen) is besloten om bij een gewicht boven de 80kg de dosering op 1dd 70mg te houden en niet standaard verder te verhogen bij (extreme) obesitas. 

ECLS/ECMO

Er zijn aanwijzingen dat de farmacokinetiek van caspofungine niet significant verandert door ECMO (ref. Wang, Borsuk-De Moor; zie bronnen). 

Verminderde leverfunctie

• Op basis van de studie van Gustot (zie bronnen) wordt geadviseerd om geen dosisreductie toe te passen bij patiënten met een verminderde leverfunctie.
• Indien er tijdens behandeling met caspofungine ernstige leverproefstoornissen ontstaan, kan worden overwogen om te switchen naar anidulafungine (ref. Jung, zie bronnen).
