In de acute fase na aspiratie is er geen sprake van een bacteriële pneumonie, maar van een chemische pneumonitis. Antibiotische behandeling is hierbij niet geïndiceerd.

amoxicilline iv 1000mg 4dd

amoxicilline po 500mg 3dd
Medication:

amoxicilline/clavulaanzuur iv 1000/200mg 4dd

amoxicilline/clavulaanzuur po 875/125mg 3dd
Remarks:

Amoxicilline/clavulaanzuur in geval van massale aspiratie.

Indications: Pneumonie t.g.v. aspiratie, hospital-acquired

Medication:

ceftriaxon iv 2000mg 1dd
Remarks:

Voeg metronidazol alleen toe in 1 of meer van de volgende gevallen:

  • massale aspiratie
  • voorbehandeling met een cefalosporine

Dosering metronidazol:

  • Indien de patiënt niet oraal belastbaar is: metronidazol iv 3dd 500 mg.
  • Indien de patiënt oraal belastbaar is: metronidazol po tablet 3dd 500 mg; metronidazol po orale suspensie 3dd 750 mg (hogere dosering dan tablet ivm lagere biologische beschikbaarheid van de suspensie).

General comments

Houd in geval van massale aspiratie rekening met eerdere (<1 jaar geleden) kolonisatie / kweekresultaten en pas de empirische therapie hier op aan.

Aanpassen therapie en IV-po switch op geleide van kliniek en kweekresultaten. 
Totale behandelduur 7-10 dagen.
