vancomycine iv loading dose 20mg/kg, 30mg/kg continuous per 24 hours
Remarks:
  • of ceftriaxon iv 2000 mg opladen, daarna 4000 mg/24u continue infusie
  • vancomycine aanpassen op geleide van spiegels*

Indications: Laat acute, hematogene infectie

vancomycine iv loading dose 20mg/kg, 30mg/kg continuous per 24 hours
Remarks:
  • Bij one-stage revisie
  • vancomycine aanpassen op geleide van spiegels*
Remarks:
  • Bij two-stage revisie: op geleide van kweken
  • Indien een cement spacer wordt geplaatst, dan antibiotica houdend cement gebruiken met clindamycine plus gentamicine, plus handmatig bijmengen met vancomycine (2000 mg vancomycine per 40 gram cement).

General comments

Behandeling van gewrichtsprothese infecties altijd in overleg met de arts-microbioloog en/of internist-infectioloog.
Bovenstaande adviezen betreffen empirische therapie (bij nog onbekende verwekker).
Indien er eerdere kweken bekend zijn, dient het empirische beleid hierop te worden afgestemd.

Bovenstaande doseringen gaan uit van een goede nierfunctie en een normaal gewicht/BMI. Bij afwijkende waarden: raadpleeg de informatiepagina per middel of neem contact op met de ziekenhuisapotheker voor advies.

In het geval van revisie chirurgie tevens antibiotische profylaxe conform antibiotica profylaxe protocol.
