bij sepsis toevoegen: tobramycine iv, startdosis 1dd 7 mg/kg aanpassen op geleide van spiegels*

Indications: Laat acute, hematogene infectie

General comments

Behandeling van gewrichtsprothese infecties altijd in overleg met de arts-microbioloog en/of internist-infectioloog.
Bovenstaande adviezen betreffen empirische therapie (bij nog onbekende verwekker).
Indien er eerdere kweken bekend zijn, dient het empirische beleid hierop te worden afgestemd.

Bovenstaande doseringen gaan uit van een goede nierfunctie en een normaal gewicht/BMI. Bij afwijkende waarden: raadpleeg de informatiepagina per middel of neem contact op met de ziekenhuisapotheker voor advies.

In het geval van revisie chirurgie tevens antibiotische profylaxe conform antibiotica profylaxe protocol:

  • BMI ≤40 kg/m2: cefazoline iv 2000mg voor chirurgische incisie
  • BMI >40 kg/m2: cefazoline iv 3000mg voor chirurgische incisie
